How to Refer your Patients
To refer your patients to us, a standard referral that includes the patient’s current medications can be uploaded to Healthlink using our Practice ID: ivymedcl.
Alternatively, standard referrals or the PDF referral form below can be sent via the following options:

Once received, our team will make contact with your patient and assess their condition to determine how IvyMed Clinic can help. Once the patient’s consultation has taken place, we will provide you with an update.
